QUESTION Are You or any other person on whom the travel plans depend, a para or tetraplegic wheelchair user AND HAVE NO other pre-existing medical conditions?

Step 2 - If your answer is No to this question or You are in doubt.
Please take time to read the DISCLOSURE OF PRE-EXISTING MEDICAL CONDITIONS below. You will need to contact Fogg Travel Medical Helpline - details are given under the DISCLOSURE OF PRE-EXISTING MEDICAL CONDITIONS.
Step 3 - Having referred Your medical condition(s) to the Helpline.
You have phoned the Medical Referral Helpline who have confirmed the level of cover provided for your condition(s). You should have been given a Medical Helpline Reference Number - Please have this number with You as You will need to insert this in the space provided.

DISCLOSURE OF PRE-EXISTING MEDICAL CONDITIONS

If at the time of applying to take out this insurance You or any person to be covered by this insurance are awaiting, or undergoing treatment as a hospital patient, or You are undergoing, or awaiting, any medical investigation/s or consultation with a specialist or awaiting diagnosis or test results, You must contact Us prior to effecting this insurance.

You will not be covered under the Cancellation or Medical Expenses sections if You fail to do so.

Please read the following:

Have you or any person to be covered by this insurance:
  • Are you a wheelchair user, other than para or tetraplegic?
  • Received treatment as a hospital in-patient or been under the care of a specialist consultant within the 12 months preceding the date of your application to take out, or renew, this insurance
  • Ever been diagnosed with, or treated for, a malignant condition or any type of cancer?
  • Been treated for a serious breathing condition within the last ten years.
  • Ever been treated for a heart related condition (including angina)?
  • Been prescribed medication for the treatment of hypertension (high blood pressure), or diabetes, within the last 12 months?
  • Suffered a stroke or required treatment for a cardiovascular condition (circulatory disorder) within the last 5 years?
  • Received treatment for, or been diagnosed as suffering from, any psychological or psychiatric disorder, including anxiety, stress or depression within the last 2 years?
  • Received any organ transplant, or are currently on dialysis?

To ascertain whether the condition/s can be included under the Cancellation and Medical Expenses Sections of this insurance and if any additional terms and/or conditions will apply. You will be entitled to cancel this insurance and receive a full refund of any premium paid if the terms and/or conditions are unacceptable to You.

If You do not disclose the condition/s, We will be unable to pay any claims relating directly, or indirectly, to the condition/s under these Sections, and Personal Accident Section of this insurance

 CHANGES IN YOUR CIRCUMSTANCES

If after taking out this Insurance You become aware of any circumstances that may give rise to a claim such as changes in your health or that of a person on whom this insurance may depend (e.g. close relative) You must contact Us and tell Us about the changes as soon as reasonably possible and prior to any trip. We may in the light of such changed circumstances alter the terms of this insurance. If the terms applied are not acceptable to You, We will cover You for any loss of deposit, or cancellation charges. You have necessarily incurred up to the date of advice to Us and that are normally covered under Section A of this insurance. In these circumstances no Policy Excess will be applied.
